Full Job Description What We Offer:

As a Case Manager MSW at Novant Health Charlotte Orthopedic Hospital you'll support our Inpatient Rehab unit as the leader of the patient assessment and complex discharge planning process. Our case managers collaborate with physicians and other team members to plan and implement safe discharge plans. Charlotte Orthopedic Hospital is is the region's only orthopedic hospital, centrally located just outside of uptown Charlotte. Our inpatient rehab unit serves adult patients for short term, multi-specialty rehabilitative care. What We're Looking For:

Education :

Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.

Experience :

Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred. Additional Skills (required): Weekend and holiday coverage as needed Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding
