Job Description

Community Health Worker - School Based Tele-Health - Rockingham County Cone Health United States, North Carolina, Greensboro Aug 25, 2026 This is a School Based Tele-Health position. The Community Health Worker works within identified communities to help patients and their families navigate and access needed medical and healthcare services, community resources, and adopt healthy behaviors. Essential Job Function Responsible for outreach to identified patients, working directly with patients in the community, in individual and group settings, connecting with patients using available forms of communication including in-person interaction to encourage health promotion and health management activities. Collaborates with patients' care team members to ensure patient is connected to resources that support overall health and helps patients establish connection to primary care provider and appointments as appropriate. Collaborates with other team members to build a library of community and healthcare resources. Assists patients navigate the healthcare system, helping the patient identify and address and eliminate barriers to accessing care. Actively participates in team meetings to provide input and feedback on patient needs and successful strategies for addressing these needs. Collects and helps analyze data from Cone Health and public data sources to identify and close gaps in care and address health disparities. Screens patients for health-related social needs and connect patients appropriately to available resources while helping patients explore their own motivation to change, identify barriers, set goals, and make progress toward improved health.
Education Required:
Completion of High School Diploma/GED.
Preferred:
Completion of North Carolina Community Health Worker Standardized Core Competency Training Program (NC
CHW SCCT
).
Experience Required:
2 years of experience in Care Management or equivalent university studies course work. Must live in or have a personal connection to the community being served or lived experience of health disparities in family or community.
Preferred:
Experience working in healthcare setting and experience health coaching within historically marginalized populations.
Licensure/Certification/Listing Required:
Valid Driver's License and must have reliable transportation to, from and between locations. Eligibility for listing as a NC Certified Community Health Worker required within one year of hire also is required.
