We are seeking a compassionate, dedicated Social Worker to join our team and help promote the well-being of our community. Position Summary The Social Worker supports the social and psychological well-being of residents and their families. This role provides education, facilitates communication, maintains accurate records, contributes to care planning, assists with medical reviews and appeals, and performs other related duties that support resident-centered care. Essential Responsibilities Provide in-service education on residents' rights, advance directives, and the psychological aspects of aging. Facilitate clear, supportive communication among residents, families, and facility staff. Maintain complete, accurate documentation in accordance with CMS, NCDHHS, DFS, and DMA requirements, including social histories, assessments, care plans, discharge plans, and progress notes. Participate in interdisciplinary care-planning meetings to support coordinated, resident-centered care. Guide and support families through medical review procedures and appeals. Attend workshops and training to remain current on best practices in geriatric care. Serve on committees, including the Interdisciplinary Care Plan Committee and the Quality Assurance and Performance Improvement Committee. Coordinate and facilitate Resident Council meetings to encourage resident participation and engagement. Provide residents and families with guidance about Medicare, Medicaid, and other assistance programs. Gather information about community health and social services and maintain a current resource file. Supervise college students completing approved fieldwork assignments and provide a supportive learning environment. Maintain regular communication with family members, encourage their involvement in resident care, and provide appropriate nonmedical updates. Advocate for residents within the facility and help ensure that their needs and rights are respected. Participate in required educational programs to support ongoing professional development. Demonstrate Trinity Oaks' & LSC's mission, vision, values, and customer service standards in daily work. Perform other duties as assigned to support facility operations.

Qualifications Education:

Bachelor's degree in social work or a related field that meets applicable state and federal requirements.

Experience:

At least one year of long-term care experience preferred.

Knowledge and Credentials:

Specialized knowledge and any licenses required by applicable regulations. Working Conditions and Physical Requirements This position requires the ability to: Bend, stoop, and lift a laptop, printed materials, and other items weighing up to 30 pounds. Move throughout the facility as needed to perform job duties. Work in an office environment, including sitting at a desk and using a computer for extended periods.
