Job Description

Social Worker - $55,000/year + credit for experience Social Workers at Maple Crest Health Center start at $55,000/year, with credit for prior experience. Work a day shift from 8:30 am-5:00 pm and make a meaningful difference in the lives of our residents and their families in Omaha. What you'll do: Conduct psychosocial assessments and provide counseling to residents and families Support admissions, transitions, and adjustment to aging or illness Work with the interdisciplinary team on care planning and documentation, including MDS Assist with discharge planning and coordination of community resources Advocate for residents and provide crisis intervention as needed Maintain accurate documentation and support financial and insurance processes Serve as a liaison with families, agencies, and mental health services What you'll need: Bachelor's degree in Social Work or a related field Current CSW certificate At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred Strong communication, critical thinking, and documentation skills Ability to work in a fast-paced, emotionally sensitive environment Proficiency with computer systems and maintaining confidentiality Ability to collaborate effectively with residents, families, and care teams Pay and benefits: $55,000/year starting, with credit added for prior experience Medical, dental, and vision insurance 403(b) retirement plan with discretionary employer match Life and AD&D insurance Short- and long-term disability coverage Accident and critical illness insurance
