Provides social services to residents of Havenwood Heritage Heights under the supervision of the Director of Care Management and in collaboration with interdisciplinary team.Compliance Statement:
Complies with all company policies. Reports instances of wrong doing, illegal and unethical behavior that occur at Havenwood Heritage Heights promptly to the Vice President of Human Resources or the President/CEO. Principle Duties and ResponsibilitiesSafety:
1.Must be committed to a high standard of safety and be willing and able to comply with all safety laws and Havenwood Heritage Heights' safety policies and rules. Must be willing to report safety violations and potential safety violations to the appropriate supervisory or management personnel. 2.Attends all mandatory in-services. 3.Wears all appropriate protective equipment (PPE) if and when in a situation that such equipment is warranted. 4.Uses proper body mechanics at all times. 5.Wears seatbelts when involved in all HHH related travel. 6.Complies with, supports and enforces Havenwood Heritage Heights' policies involving all safety procedures to also include the proper use of body mechanics, mechanical lifts and infection control.Care Management:
1.Provides social services to residents in assigned Havenwood Heritage Heights Community (HHH). Responsible for discharge/transfer planning as well as participating in care planning for all assigned cases in the Havenwood Heritage Heights Community. Serves as a contact and a resource to the family members of residents that they are assigned. Rev. 3/2019 1 2.Provides back up coverage for Health Services Center Admissions coordinator and other Social Workers as assigned. 3.Assesses assigned residents for psycho-social needs, and works with the Finance office to determine financial payor source. 4.Advocates for Resident Rights, promoting empowerment and self-determination while enabling resident to receive needed care. 5.Complies with all state and federal documentation regulations and Havenwood Heritage Heights' policies and procedures regarding documentation. 6.Promotes and maintains good working relations and communications with customers, internal and external. - Masters in Social Work and a solid body of knowledge in Medical Social Work (Exempt).
- BSW with three to five years' experience in a health care setting required (Non-Exempt).
- Excellent assessment, observation and documentation skills necessary.
- Ability to work autonomously and to develop a clear understanding of state and federal regulations pertaining to the social worker's field of practice is expected.
- Must be able to work well with others in an interdisciplinary team effort.
