Position Summary Under the supervision of the Supervisor - R4R, the Family Case Manager provides comprehensive case management and support services to individuals and families affected by co-occurring mental health and substance use disorders. This role focuses on promoting recovery, family stability, and overall well-being through coordinated care, advocacy, and skill development. Working collaboratively with a multidisciplinary team, the Family Case Manager develops individualized service plans that address the unique needs of each client and family system. The position also assists individuals in accessing necessary resources and navigating complex social service and health care systems. Additionally, the Family Case Manager provides intensive system coordination to address participants' whole health needs, including medication-assisted treatment (MAT), chronic and acute health conditions, and mental health concerns. Essential Functions Provide comprehensive case management and support to individuals and families enrolled in Family Case Management services promoting recovery, family stability, and overall well-being. Services can include individual support or group support using the CRAFT model. Collaborate with clients and program staff to develop individualized recovery and service plans that reflect personal goals and address mental health, substance use, and social support needs. Assess client needs across multiple domains—including mental health, substance use, physical health, housing, and financial stability—while identifying strengths and barriers to recovery. Offer brief support, psychoeducation, and guidance to clients and families to enhance understanding of co-occurring disorders, treatment options, and available community resources. Assist individuals and families in navigating social service and health care systems, helping them access necessary supports such as housing, medical care, and benefits. Coordinate referrals for services within and outside the Center, including specialty behavioral health care, substance use treatment, primary care, housing assistance, and vocational programs. Collaborate with community and system partners—including correctional facilities, DMHAS Interim Managing Entity (IME), and other agencies—to ensure continuity and coordination of care. Participate in case reviews, supervision, and multidisciplinary team meetings to monitor client progress and adjust service plans as needed. Maintain timely, accurate, and compliant documentation in accordance with Center policies, state regulations, and Joint Commission standards. Perform all other job-related duties as assigned to support program and agency goals. Qualifications Essential Bachelor's degree in psychology, counseling, social work, criminal justice, education, or other behavioral health related field Must have relevant experience providing services and case management to individuals living with a substance use disorder Has experience in working across cultures Must possess knowledge of formal/informal community systems and resources Must have knowledge of Wellness and Recovery Principles, Person Centered Care, Trauma Informed Care and Integrated Healthcare Must possess a valid NJ driver's license in good standing Ability to work collaboratively and build relationships with colleagues, coworkers, vendors, contractors, and the public Preferred

CADC/LCADC

Bilingual (Spanish/English), and experience working with culturally diverse populations Knowledge of community resources and services and the ability to collaborate with other professionals Trained in conducting Substance Use Interventions Advanced degree in social work, public health, or a related field preferred

Salary:

$55,000-$70,000/year Benefits include health coverage (medical, prescription, and dental), paid time off (vacation, holidays, and sick), a deferred compensation plan, and enrollment in the NJ State Pension System.
