What You'll Do:
Provide direct clinical social work services, including assessment, consultation, therapy, and client-centered advocacy Interview clients, families, and involved agencies to gather social/clinical data for diagnosis and treatment planning Conduct assessments and develop plans that enhance social functioning and support community reintegration Engage clients therapeutically to address mental health symptoms and coping challenges in the social environment Maintain an up-to-date directory of resources (health care, social services, housing, entitlements/disability, vocational supports) Make and coordinate appropriate referrals/linkages, updating resources monthly or as needed Engage with referral source representatives as requested to support continuity of care Prepare reports with findings, conclusions, and recommendations as required by funding sources Document employment/treatment progress in permanent client files and collect required data Participate in required internal/external meetings, trainings, supervision, and staff conferences Maintain strict confidentiality and comply with administrative policies and procedures Complete other duties as assignedWhat You Need:
New Jersey Licensed Clinical Social Worker (LCSW) with 2 years of appropriate experience Working knowledge of Social Learning Theory, Cognitive-Behavioral interventions, and clinical assessment/evaluation Excellent written and verbal communication skills Outstanding organizational skills with the ability to multi-task and manage priorities Valid driver's license and ability to meet agency driving standards Ability to pass a criminal background check Ability to handle Protected Health Information and comply with HIPAA Privacy and Security Rules Bilingual abilities are a plusWhy This Role Matters:
