Job Description

Join our compassionate team and make a meaningful impact in post-acute and long-term care. We are seeking an experienced Licensed Clinical Social Worker (LCSW) to provide mental health services to residents in a skilled nursing facility. You'll work alongside an interdisciplinary team to support residents' emotional well-being, improve quality of life, and promote positive clinical outcomes. Responsibilities Conduct psychosocial and mental health assessments. Provide individual and family counseling, crisis intervention, and behavioral support. Develop and implement individualized treatment plans. Collaborate with physicians, nursing, therapy, and facility staff. Maintain accurate, timely documentation in the EMR. Support residents coping with adjustment, depression, anxiety, dementia, and other behavioral health needs. Qualifications Current LCSW license Skilled nursing, rehabilitation, long-term care, or post-acute experience preferred. Strong assessment, counseling, and care coordination skills. Experience with EMR documentation and interdisciplinary care teams.
