Job Description

Sr Service Coord Early Intervention Position Summary Responsible for provision of Service Coordination for Hunterdon County children in the Early Intervention Program; assists in ensuring compliance to state and regional policies regarding EIS. Assists in supervising Service Coordination staff. Primary Position Responsibilities 1. Communicates with each family from Hunterdon County whose child is found eligible for EIP Services. 2. Facilitates the provision of appropriate comprehensive services for infants and toddlers with developmental delays related to Statewide Early Intervention system. 3. Completes HMC, regional, and state documentation. 4. Transitions child to appropriate services upon discharge from Early Intervention Program. 5. Ensures Administrative responsibilities of position are met.
Qualifications Minimum Education :
Required:
Bachelor's Degree in
Social Work or Nursing Preferred:
Master's Degree in Social Work or Nursing Minimum Years of Experience (Amount, Type and Variation) :
Required:
Five (5) years experience in public health and/or pediatrics (hospital, clinic, EIP, etc.) including supervisory experience or a Master's Degree and three (3) years experience in public health, and/or pediatrics with some supervisory experience.
Preferred:
Experience with special needs children
License, Registry or Certification :
Required:
New Jersey RN licensure or New Jersey Social Work licensure. Valid driver's license
Preferred:
None Knowledge, Skills and/or
Abilities :
Required:
A car to transport self throughout the county and region. Ability to flex schedule in order to accommodate family needs. Knowledge of child development, developmental delays such as autism, Down Syndrome, Cerebral Palsy, etc., parent-child interactions, infant mental health issues. Understanding the field of rehabilitation and education and the role of various professionals. Knowledge of local and statewide social service resources.
