As a Social Worker , you will play a vital role in revolutionizing care delivery at our 174-bed center, which has been ranked as a "Best Nursing Home" by U.S. News & World Report. About the
Role:
As a Social Worker at Allaire Rehab & Nursing , you will work closely with our interdisciplinary team to provide exceptional care and support to our residents. You will assess the social, emotional, and psychological needs of our residents, develop and implement personalized care plans, and facilitate family support and education. Your goal will be to ensure that our residents receive comprehensive care that addresses their physical, emotional, and spiritual well-being.
Responsibilities:
  • Conduct initial and ongoing assessments of residents' social, emotional, and psychological needs
  • Develop and implement personalized care plans in collaboration with the interdisciplinary team
  • Provide counseling and support to residents and their families to address emotional, social, and psychological concerns
  • Facilitate communication and coordination with healthcare providers and community resources to ensure seamless transitions and care
  • Participate in care plan conferences and contribute to the development of resident-centered care plans
  • Maintain accurate and thorough records of resident assessments, care plans, and services provided
