This position involves meeting with patients, speaking to families and outside agencies, documenting in the electronic medical record, and assisting LSWs and LCSWs in making referrals. This is a fast-paced environment and requires incredible attention to detail. The Discharge Planner will work independently, when necessary, as well as part of the Social Work team under the direction of the unit's Social Workers.
