Part-time Social Worker (LSW / CSW / LCSW) - Skilled Nursing & Rehab Make a Meaningful Difference Every Day Complete Care at Madison is seeking an experienced and compassionate Social Worker (LSW / CSW / LCSW) to join our skilled nursing and rehab team. We are looking for a social worker with long-term care and rehabilitation experience who is confident navigating admissions, discharge planning, family support, and interdisciplinary care coordination.
