Adult medical day care center in Newark is looking to hire a social worker who is passionate about recruiting and working with older and disabled adults to ensure that our clients receive the highest quality of care possible and are treated with dignity and respect. Recruiting is our priority and a multifaceted approach is needed. This position will report to and be supervised by the Center Director.

Duties and Responsibilities include:

Recruit new clients for adult medical day care through home and senior housing visits, site visits with local physicians and area hospitals, establish relationships with discharge planners at all area hospitals and rehab centers. Oversee activities in the social work department including training, scheduling, supervising, and evaluating department staff as required. Conduct and coordinate on-site and home visits, intake interviews, client assessments, evaluations, summaries, and other admission functions. Coordinate with internal and external ancillary services to provide quality care. Ensure compliance with all company policies and procedures and applicable regulatory authorities (i.e. NJ Medicaid, State and Federal Environmental Agencies, OSHA, USDA) and prepare paperwork as required. Participate in Safety and Quality Assurance Committees, Caregiver Support Groups, and seminars. Participate in the care plan for interdisciplinary team and record clinical notes as required. Maintain formal written communications with clients and families and facilitate counseling through support groups and one-on-one sessions. Maintain accurate files and records of clients and social services information. Provide billing, transportation, and client referral information as needed. Participate in community outreach programs, including communication with other health and social services providers. Participate in Center marketing activities. Provide active assistance with clients as needed (i.e. hygiene, meals, ambulation, arrivals and departures, and social activities).

QUALIFICATIONS
