Axia Care Center 135 South Center Street Orange, NJ 07050 Axia Care Center is seeking an experienced Director of Social Services to lead our Social Services Department. ?

SKILLED-NURSING EXPERIENCE IS REQUIRED

Candidates must have: At least two years of recent Social Services experience in a skilled nursing or long-term-care facility Active New Jersey LSW or CSW Hands-on experience with MDS documentation Hands-on experience completing and managing POLST forms Experience investigating, documenting, and resolving resident and family grievances Experience with care plans, discharge planning, advance directives, resident rights, and regulatory compliance Previous leadership or supervisory experience preferred

Key Responsibilities:

Direct the daily operations of the Social Services Department Complete required MDS documentation accurately and on time Manage POLST forms, advance directives, and healthcare decision-making documentation Investigate and follow up on resident and family grievances Participate in care-plan and interdisciplinary team meetings Coordinate safe discharge planning and community services Address psychosocial, behavioral-health, and resident-rights concerns Maintain compliance with federal and New Jersey skilled-nursing regulations Supervise Social Services staff and ensure timely documentation ️ Applicants without direct skilled-nursing or long-term-care experience will not be considered for the Director position.
