As a Social Worker/Care Companion, you'll play a vital role in our Collaborative Care team, providing emotional support and mental health assessments to residents in skilled nursing facilities. About the Role As a Social Worker/Care Companion, you'll visit designated facilities, conduct assessments of residents, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. You'll also document visit notes and maintain detailed records of interactions and interventions after each visit. Additionally, you'll collaborate with psychiatric professionals to ensure comprehensive care. Key Responsibilities Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Qualifications Associate, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills
