Part-Time Social Worker Preferred Care at Wall - Wall Township, NJ Preferred Care at Wall is seeking a compassionate and dedicated Part-Time Social Worker to join our skilled nursing and rehabilitation team. The ideal candidate will support residents and their families throughout the admission, care-planning, and discharge process while promoting each resident's emotional well-being, dignity, and quality of life.
Responsibilities:
Complete psychosocial assessments and required documentation. Participate in resident care-plan meetings. Provide emotional support, counseling, and advocacy to residents and families. Assist with admissions, transitions, and discharge planning. Coordinate community resources, services, equipment, and follow-up care. Address resident and family concerns promptly and professionally. Support residents' rights and ensure compliance with facility policies and state and federal regulations. Collaborate with nursing, rehabilitation, admissions, and other interdisciplinary team members. Maintain accurate, timely, and confidential resident records.
Qualifications:
Bachelor's degree in Social Work or a related human-services field required. Previous experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred. Knowledge of discharge planning, community resources, and resident-rights requirements. Strong communication, organization, documentation, and problem-solving skills. Compassionate, dependable, and able to work effectively with residents, families, and staff.
