A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description
What You'll Do:
Perform psychosocial assessments for hospice patients and families/caregivers in a timely manner Help ensure patient and family needs are addressed within the plan of care Provide counseling and emotional support related to stress, coping difficulties, grief, and end-of-life care Coordinate community resources and assist with applications for Supplemental Security Income, Medicaid, Hospice, and Medicaid Waiver programs Serve as a liaison between patients/families and community agencies Maintain accurate and timely documentation Collaborate with the interdisciplinary hospice team to provide exceptional patient care and customer servicePay & Benefits:
