What You'll Do As an LMSW with Horizon, you will provide behavioral health services to older adults at assigned facility locations within your geographic area.
Responsibilities include:
Provide individual psychotherapy to adults age 55+ Conduct clinical assessments and develop individualized treatment plans Support patients experiencing anxiety, depression, grief, loss, adjustment concerns, health challenges, and life transitions Utilize evidence-based therapeutic interventions Develop meaningful and consistent therapeutic relationships Maintain timely and accurate clinical documentation Collaborate with facility staff and multidisciplinary clinical teams Participate in clinical supervision and professional development Maintain professional, ethical, and regulatory standards of care What We're Looking For Required Qualifications Master's degree in Social Work from an accredited program Active New York State LMSW license Clinical experience working with adults age 55+ Strong clinical assessment and communication skills Excellent organizational and documentation skills Compassionate, patient-centered approach to care Ability to work independently while collaborating with a clinical team Reliable transportation and ability to travel locally to assigned facility location(s) Preferred Experience Geriatric or older-adult behavioral health Skilled nursing or rehabilitation Senior living or long-term care Healthcare-based behavioral health Treatment of depression, anxiety, grief, loss, and adjustment concerns What Horizon Offers We want our clinicians to feel supported professionally and personally.
