Job Description

Make a Difference Every Day The Service Coordinator is a relationship-centered position for someone who enjoys helping people navigate challenges, connect with resources, and build a stronger sense of community. You will serve as a trusted resource and advocate for residents while connecting them with services that support health, independence, well-being, and quality of life. You will build relationships with residents, families, community organizations, and service providers; identify resources; coordinate programs; and help residents access benefits and services that meet their needs. What You'll Do  Build trusting relationships with residents and provide basic case management and service coordination.  Connect residents with in-house resources and community agencies that can help meet their individual needs.  Help residents understand and access benefits and programs for which they may be eligible.  Serve as a liaison between residents, management, families, and community service providers.  Develop and maintain a resource directory of local and state agencies and programs.  Identify new services, programs, and community partnerships that can benefit residents.  Coordinate on-site educational, health, wellness, and community programs and events.  Monitor participation in outside services, identify barriers to access, and look for ways to improve engagement.  Encourage residents to build informal support networks with family, friends, and fellow residents.  Promote resident participation in activities and programs that strengthen community and connection.  Advocate for residents and help identify opportunities for additional support or services.  Assist residents with forms, service applications, mail, notifications, and other information they may need help understanding.  Share consumer information and resources from state and local aging agencies, AARP, senior centers, councils on aging, legal services, Medicare, Medicaid, and other programs.  Attend regularly scheduled in-service and continuing education programs as required.  Submit reports to management and HUD as required. What We're Looking For  Associate's Degree in Human Services, Social Work, Health Care Management, Psychology, Case Management, or a closely related field required.  One year of experience in social services delivery with an elderly and/or family population, or other directly related human service experience.  Experience working in nonprofit settings and with elderly and/or disabled populations preferred.  Ability to communicate in English in both oral and written forms and follow oral and written directions.  Basic computer skills.  Compassion, patience, resourcefulness, organization, and a strong commitment to excellent service. Physical & Work Environment The physical demands are representative of those required to perform the essential functions. Reasonable accommodations may be made for individuals with disabilities. The position may occasionally require standing, walking, sitting, reaching, climbing stairs, kneeling, stooping, crouching, talking, hearing, and related activities. The employee may occasionally lift or move up to 25 pounds. The work environment is indoors with a usually moderate noise level and no routine exposure to weather conditions. Annual continuing education requirements for the Service Coordinator position, as outlined by HUD, must be fulfilled.
