Role:

As a social worker, you'll work with a care team that includes a physician or nurse practitioner, to assess mental and behavioral needs, develop treatment plans, and collaborate with other medical staff to ensure comprehensive care for a unique patient population.

Responsibilities include:

Oversees and provides direct behavioral health care with supervision Facilitates care coordination and clinical collaboration with supervision Ensures quality care and risk mitigation with supervision You will be primarily based at our Blasdell location, 4233 Lake Ave, Blasdell NY 14219, and will travel to other Neighborhood sites in Western New York as needed. Compensation:

LMSW - $62,000-$66,000; annual salary (based on a full-time, 40 hour work week) Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
