Role:

As a social worker-Healthy Steps specialist, you'll work with a care team that includes a physician or pediatric nurse practitioner, and consult with families with babies and young children regarding developmental and behavioral concerns.

Responsibilities include:

Conduct team-based well-child visits with parents and their babies and young children with a physician/pediatric nurse practitioner Oversees and provides direct and team-based behavioral health care with supervision Facilitates care coordination and clinical collaboration with supervision Ensures quality care and risk mitigation with supervision You will be primarily based at our Northwest location, 155 Lawn Ave. in Buffalo, and will travel to other Neighborhood sites in Western New York as needed. • 72,000 annual salary (based on a full-time, 40 hour work week) Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
