Social Worker Skilled Nursing Facility Full-Time Salary:
Up to $75,000 per year Make a Difference Every Day We are seeking a compassionate and dedicated Social Worker to join our interdisciplinary team at our Skilled Nursing Facility. This is an excellent opportunity for a caring professional who is passionate about advocating for residents, supporting families, and helping improve the quality of life for those in our care. As a Social Worker, you will play a key role in providing emotional support, coordinating care, and ensuring residents receive the resources and services they need throughout their stay.
Responsibilities:
Conduct psychosocial assessments upon admission and as needed Develop and implement individualized care plans with the interdisciplinary team Provide counseling and emotional support to residents and their families Coordinate discharge planning and facilitate smooth transitions of care Assist residents and families with community resources and long-term care planning Advocate for resident rights and well-being Participate in care plan meetings and family conferences Maintain accurate documentation in accordance with federal, state, and facility regulations Collaborate with physicians, nursing staff, and other healthcare professionals to ensure the highest quality of care
Qualifications:
Bachelor's Degree in Social Work (BSW) required; Master's Degree (MSW) preferred Previous experience in a skilled nursing, rehabilitation, or long-term care setting preferred Knowledge of discharge planning and long-term care regulations Excellent communication, interpersonal, and organizational skills Compassionate, dependable, and resident-focused Ability to work effectively in a fast-paced, team-oriented environment
