Atlantic Dialysis Management Services is looking for a Full time Licensed Master Social Worker to work at our Manhattan Dialysis Center. Provides social case work and support to the ESRD patient and his/her family, so that they maintain a meaningful lifestyle within the limitation of the illness and treatment.
RESPONSIBILITIES
Conducts psycho-social assessments on each patient and determines social work treatment plan compatible with a total medical treatment plan. Responsible for knowledge of the benefits available to patients under Public Law 92-603, Section 2991 for Medicare, as well as Medicaid, Blue Cross and other insurance coverage. Assists patients with interpretation of benefits available to these patients and assists patients in obtaining the benefits to which they are entitled. Participates in multi disciplinary conferences and long-tern, short-term care plan of the patient. Utilization of crises intervention techniques in accordance with assessed individual and/or family needs. Uses psychiatric consultation in selected situations. Refers to appropriate public and private community agencies and assists patients and their families in the use of them: (i.e.) transportation, public assistance, vocational rehabilitation, etc. Liaison between the Dialysis Center and affiliated hospitals and other dialysis center, and collects pertinent information for patients transfer or transient visit to other hospital or dialysis centers. Requirements Must have experience working with dialysis patients Master's Degree in Social Work with a specialization in clinical practice from an accredited School of Social Work and Certification from the New York State Education Department. Must be Licensed Qualified by credentials and experience to meet Federal regulations for Social Workers in ESRD Programs.
