Full Time SUMMARY:

The Outpatient Social Worker provides customized clinical interventions, that are evidence-based, to treat underlying, root causes of psychosocial and environmental symptoms. Serves as a liaison between medical professionals and patients to increase collaboration and positive medical outcomes.

RESPONSIBILITIES

Provides crisis intervention with an immediate safety action plan for patients with suicidal, homicidal, hallucinating, substance abuse, and interpersonal violence/abuse issues. Provides Social Work assessments that identify the underlying root cause of psychosocial and environmental symptoms. Creates a care plan and links patient with services to address the needs of the patient. Manages referrals regarding psychosocial needs and determines appropriate interventions and strategies to meet those needs. Facilitates behavioral change through the use of Clinical Interventions such as but not limited to; Motivational Interviewing, Collaborative Care, Cognitive Behavioral Therapy, Behavioral Activation, and Problem-Solving Therapy. Creates customized interventions for specialty populations. Completes diagnostic screening tools such as the PHQ screenings, Edinburgh, CAGE, and GAD 7. Diagnoses patients using the DSM V. Documents all services in accordance with clinical guidelines and policies.

REQUIRED QUALIFICATIONS

Social Work Required Certification/Licensure:

NYS LMSW or

NYS LCSW PREFERRED QUALIFICATIONS

One (1) to three (3) years of social work experience in a health care or home care setting End renal stage disease experience (dialysis) Compassionate, warm and patient focused Exceptional documentation and planning skills Excellent communication and interpersonal skills

Minimum one (1) to three (3) years pediatric and/or palliative care experience

PHYSICAL REQUIREMENTS

Sedentary

  • Sedentary roles are primarily office-based and require prolonged sitting (67-100% of the workday) with minimal physical effort and lifting limited to under 10 lbs.

Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver's license.

PAY RANGE

$62,500.00

  • $78,250.

00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
