Shift Details:

Monday-Friday 8:00am-4:30pm

Social Worker Duties & Responsibilities:

Assessing individuals, developing individual care plans and determining necessary resources. Advocating for and assisting individuals and families in accessing and obtaining services. Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals. Monitoring individual rehabilitation goals and progress. Providing crisis intervention services. Minimum Qualifications Current, valid, and unrestricted NY license/certification in good standing, as required for the position. Minimum of one (1) year related experience Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs. Valid drivers license. Additional certifications may be required.
