A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

In this consultant role, you will work directly with people we support in their homes and day programs, helping promote emotional well-being, personal growth, independence, and quality of life while collaborating with an interdisciplinary team.

Key Responsibilities Provide social work consultation and support to adults with intellectual and developmental disabilities. Meet with people we support in their homes and day program settings. Support emotional well-being, coping skills, self-advocacy, and personal growth. Collaborate with program staff, families, clinical team members, and interdisciplinary team members. Assist with identifying needs, developing recommendations, and supporting individualized goals. Participate in team meetings and service planning, as appropriate. Maintain professional documentation in accordance with agency policies and applicable standards. Provide services in a professional, ethical, and person-centered manner. Requirements Current New York State Licensed Master Social Worker license required Experience working with adults with intellectual and developmental disabilities preferred Strong assessment, communication, documentation, and interpersonal skills Ability to work independently while collaborating with an interdisciplinary team Valid NYS driver's license and reliable transportation required Ready to Apply?
