POSITION SUMMARY :
The Bereavement Social Worker is responsible for the creation of bereavement plans pertinent to each family to support their grief through an interdisciplinary approach. The Bereavement Social Worker will initiate bereavement plans of care for bereaved family members and provide follow-up supports for 13 months following the patient's death. Supports may include phone calls, one-on-one counseling (in homes, virtually or in our Hospice Grief Center), facilitating support groups and other grief-related activities, developing an annual service of remembrance and additional community events throughout the year. Bereavement Social Workers will also have the opportunity to participate in interdisciplinary group meetings to gain knowledge of patients' and families' anticipated grief.
Minimum Education:
A successful candidate must have an LMSW from a Social Work program accredited by the Council on Social Work (CSWE); L.M.S.W., L.C.S.W., or eligible.
Minimum Experience:
Six to 12 months of post-Master's experience in grief counseling, Hospice social work preferred.
