Location:
Long Island City, Queens, NY We are seeking a compassionate and dedicated Social Worker to provide trauma-informed, culturally responsive mental health services to transgender and gender non-conforming adults residing at our supportive housing program in Long Island City, Queens. This role is ideal for someone who is passionate about empowering underserved communities and helping clients achieve greater stability, wellness, and independence. Key Responsibilities Conduct client intakes, psychosocial assessments, and determine service needs. Develop individualized service and treatment plans that support clients' mental, emotional, and social well-being. Provide counseling and supportive interventions based on clients' needs and within the scope of practice. Coordinate and monitor referrals to medical, behavioral health, mental health, and other community-based services. Maintain regular communication with clients' support systems, including family members, caregivers, and other service providers, as appropriate. Provide ongoing assessment, case planning, and monitoring related to clients' social, behavioral health, trauma, and mental health needs. Assist with crisis intervention and de-escalation as needed. Prepare timely and accurate case notes, progress reports, evaluations, and other required documentation. Participate in quality assurance and program evaluation activities. Collaborate with interdisciplinary staff and external partners to ensure coordinated, client-centered care. Perform other duties as assigned by the Supervisor. Minimum Qualifications Master's degree in Social Work, Psychology, Human Services, or a related field required . Minimum of 1-2 years of relevant experience in case management, social services, behavioral health, or a related setting. Experience working with individuals impacted by homelessness, poverty, mental health challenges, chronic health conditions, and diverse cultural and linguistic backgrounds is required. Experience serving LGBTQ+ communities, particularly transgender and gender non-conforming individuals, is strongly preferred. Demonstrated commitment to trauma-informed, person-centered, and culturally affirming care.
Pay:
$65,000.00 per year
