Job Summary:
We are seeking a compassionate and skilled Social Worker Assistant to join our team at North Country Nursing and Rehabilitation Center. As a Social Worker Assistant, you will work closely with our social work team to provide supportive services to our residents, focusing on maintaining their emotional and psychological well-being. This is a PART_TIME opportunity for a motivated and caring individual who is dedicated to making a positive impact in the lives of others.
Responsibilities:
Assist social workers in conducting assessments, developing and implementing care plans, and providing emotional support to residents and their families Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improving outcomes Facilitate activities and programs designed to promote resident engagement and socialization Collaborate with other healthcare professionals to ensure comprehensive care and support for residents Maintain accurate and timely documentation of services provided and resident progress
