The Case Manager works collaboratively with families to promote stability, self-sufficiency, and long-term housing success while maintaining a trauma-informed and client-centered approach. Key Responsibilities Conduct intake assessments with new residents to identify strengths, challenges, and immediate needs. Develop individualized service plans with measurable goals focused on housing, employment, education, health, and family well-being. Provide regular case management sessions to monitor progress, offer support, and adjust plans as needed. Link families to appropriate community resources including housing programs, childcare, mental health services, substance use treatment, public benefits, and legal aid. Advocate on behalf of residents with service providers, landlords, schools, and other systems as needed. Assist families with housing searches, applications, and navigating the transition to permanent housing. Maintain accurate and timely case notes, assessments, and documentation in accordance with agency and funder requirements. Participate in multidisciplinary team meetings and collaborate with shelter staff to ensure coordinated care. Provide crisis intervention and emotional support when residents are experiencing challenges. Ensure residents are informed of shelter rules, policies, and available resources in a respectful and empowering manner. Maintain confidentiality and uphold professional boundaries at all times. Qualifications Associate's or Bachelor's degree in social work, human services, psychology, or a related field (Master's degree preferred). Minimum of 2 years of case management or social service experience, preferably working with homeless or vulnerable families. Knowledge of housing systems, public assistance programs, and trauma-informed care. Strong interpersonal, communication, and organizational skills. Ability to work independently and manage multiple priorities in a fast-paced environment. Proficiency in documentation and use of electronic case management systems. Bilingual (English/Spanish or other relevant language) preferred but not required. Valid driver's license and reliable transportation preferred. Working Conditions Combination of office-based work and direct interaction with residents. May require occasional evening or weekend hours. Must be comfortable working in a shelter environment with families in crisis.

From $18.00 per hour Expected hours: 40.0 per week
