Niagara Falls Memorial Medical Center is seeking a master's-prepared Social Worker to join our fast-paced Emergency Department team. Natura and Scope:

The Emergency Department Social Worker provides comprehensive patient-centered assessments, management and support to patients and their families at the Emergency Department of Niagara Falls Memorial Medical Center. The position will work with both behavioral health and medical patients who utilize the Emergency Department. Duties will include the completion of patient assessments utilizing the tool featured in the Cerner system, development of appropriate care plans, counseling and interventions to meet the physical health, behavioral health and social determinant of health needs of patients and families. Position will place a special emphasis on the preparation of assessments and care plans for high utilizers of emergency department services. Position will initiate and implement advance directives, as needed. In addition, the Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual patient's physical, behavioral health and social determinant of health needs. Individual holding this position must possess strong communication skills, have knowledge of available community resources, document all interventions pursuant to the highest social work standards, participate in departmental meetings, in-services and educational programs , engage in quality improvement activities and perform other relevant duties as assigned to including providing coverage for other social workers and back-up responsibilities.

Education and Experience:

Required:

A Master's degree in Social Work, MSW or LMSW and relevant post-master's experience, preferably in a healthcare environment; or a Masters in Mental Health Counseling with relevant experience working in healthcare setting. An individual who actively enrolls in a Master of Social Work program at an accredited college or university within six months of his/her hiring will meet these educational requirements, providing the individual can demonstrate graduate school attendance and ultimately graduate from the aforesaid program.

Founded in 1895, Niagara Falls Memorial Medical Center (NFMMC) has a long tradition of serving the Niagara community. NFMMC's hospital and primary stroke program are accredited by the Accreditation Commission for Health Care, and its primary care network is accredited by the National Committee for Quality Assurance. Memorial's inpatient services include a cardiac/stroke unit, a medical-surgical unit, an ICU, a Labor and Delivery unit, and an Adult Inpatient Psychiatry unit, the only one of its kind in Niagara County.
