About Us:
Champlain Valley Family Center is a leading provider of integrated behavioral health services in Plattsburgh, NY. As a Certified Community Behavioral Health Clinic (CCBHC), we are committed to providing comprehensive, person-centered care to individuals and families in our community.
Program Description:
The Intense Family Support Program is an intensive youth family counseling program. The primary goal is to keep high risk youth out of placement, and assist students to be successful in school with the support of family/guardians and the School District Personnel. CVFC has a comprehensive benefits package, including Health Insurance, Dental, FSA, AFLAC, Life Insurance, Short Term Disability, Vision, PTO and Mileage Reimbursement for work related transportation in a personal car.
Responsibilities:
Communicate with interested parties including schools, parents/families, legal entities, and social service representatives to provide support and case management. Providing clients and parent/guardians with working behavioral plans and modify them as needed Provide extracurricular activities for clients (Biking, hiking, sports, etc.) Assisting clients to participate in various school, community and social activities. Will be providing transportation to kids and their families at times. Assist clients in court or justice related mandates. Maintain on going contact reports and contact checklists. Keep up to date records on expenses occured each month and be accountable for balances.
Qualifications:
Bachelor's Degree is
REQUIRED
for this position. Knowledge of community resources is preferred.
