Overall responsibilities includes, ensuring timely access to prenatal care, SNAP (food stamps), completing referrals to the WIC Program, and linking family to appropriate internal and external resources as needed. The FSW/PN coordinator also provides services that consists of the clinic psychosocial referrals, provide short term interventions and education on pre and post-natal care. A key component of the position is to coordinate services with the medical and the community resources, as well as collaboration with outside service providers. Qualifications Education and/or Experience - Preferred a Bachelor's Degree in Social Work, Public Health, Human Services, or related field; or associate degree with 2 years of relevant work experience providing case management services to child/youth and families. Must possess knowledge of community resources and the ability to communicate effectively in English and Spanish. A working knowledge and understanding of patient/client confidentiality issues and HIPAA compliance/regulations are essential. Requires some travel to community partners and monthly workshops.
