A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

Job Description
The Connector Support was developed in response to a need identified to support students throughout the school day and to enhance the Connected Community Schools Initiative. Connectors will be present throughout the building to engage with students, faculty & staff; creating positive relationships upon arrival through dismissal every school day. Connectors will be present during bus arrival and departure at the end of each school day. Connectors will engage and support students during lunch, study hall and other opportunities that warrant additional support. Connectors will collaborate with school administrators, Social Workers, Counselors, Connected team and any additional school staff to ensure support is provided throughout the school day and to foster positive school connections. Connectors will ensure that students are following the school rules and maintaining a safe environment in the hallways, cafeterias, bathrooms, classrooms etc. Connectors will support school teams to identify and respond to the needs of students. Connectors will ensure students that are in need of resources, services, tangible items are connected to those supports and places within the building.(I.e. Nurse's office, Counseling office, HUB etc.). Connectors will be proactively identifying students in need of social, behavioral, and mental health support, and connect those students with opportunities and support within the school building, also known as the LINK team.
