Full-Time At Hometown Health Centers , we believe whole-person care means supporting not only our patients' physical health, but their emotional, behavioral, and social well-being. We are seeking a compassionate and culturally responsive Social Worker to join our multidisciplinary team and make a meaningful difference in the lives of individuals and families in our community. This position provides counseling, assessment, advocacy, care planning, crisis support, and connection to community resources for patients facing social, emotional, behavioral, and economic challenges. What You'll Do Provide assessment, diagnosis, and treatment of mental, behavioral, and emotional health concerns for primary care patients. Assess patients' needs, strengths, goals, and support systems. Develop individualized treatment plans and monitor patient progress. Provide counseling and support to help patients navigate life changes and challenges. Connect patients with community resources and social service providers while advocating for their needs. Respond appropriately to crisis situations both in person and by phone. Maintain accurate, timely, and confidential clinical documentation and treatment plans. Collaborate with family members, collateral contacts, and members of the care team when appropriate. Work closely with a multidisciplinary team to provide coordinated, patient-centered care. Practice with cultural humility, empathy, professionalism, and respect for patients from diverse backgrounds.
What We're Looking For Education & Licensure:
Master's degree from an accredited program in Social Work (MSW), Clinical Mental Health Counseling (MHC), New York State licensure as LMSW, LCSW, LMHC, or LMFT . Clinical supervision is available.
Experience & Skills:
Experience working with underserved, low-income, and culturally diverse populations. Strong understanding of community resources and social service systems. Clinical experience in a primary care setting is a plus. Experience with Electronic Health Records (EHR), Microsoft Office, and Outlook preferred. Excellent communication, interpersonal, and crisis-management skills. Ability to build trust and establish meaningful relationships with patients. Bilingual (English/Spanish) preferred. Additional Requirements Valid New York State driver's license and reliable access to a vehicle. Ability to travel occasionally between our Schenectady and Amsterdam offices and School-Based Health Centers. Ability to sit and work at a computer for extended periods, as well as stand and walk as needed. Ability to move/lift up to 25 lbs. Why Join Hometown Health Centers?
