Temporary Wyoming County Community Healthy System:
Skilled Nursing Facility is looking to hire one (1) Per Diem/Temporary Social Work Assistant. This position requires the performance of social work services in helping clients with social, emotional and related difficulties associated with their medical conditions. The work will be performed under the supervision of the Director of Nursing who consults with a certified Social Worker.
Typical Work Activities:
Provides social work services to clients to assist them in their adjustment to their illnesses, disabilities and placement; Collects and maintains appropriate data from various sources such as professional staff and residents' families to establish and overall approach to solution of medical and social problems; Maintains information on community and health resources which can be utilized during the clients' care and/or subsequent to the residents' discharge; Obtains and records social information to develop client case histories and updates; Interviews and evaluates prospective admissions explaining requirements and policies; Participates in the development and implementation of and adherence to the rights and responsibilities of residents; Participates in the process of transferring residents to appropriate levels of medical care; Coordinates and attends interdisciplinary conferences weekly to review care plans and ensure a concise care plan is in effect; Manages residents' personal affairs such as paying bills, banking, purchasing personal needs and paperwork; Schedules, organizes and leads family group and resident group meetings, including arranging for speakers and special guests; Refer residents and family for appropriate community services.
Minimum Qualifications:
1. Graduation from a regionally accredited or New York State Registered college or university with a Bachelor's Degree in Social Work or closely related field, or 2. Four (4) years of Social Work experience in a long term care facility in New York State; or 3. An equivalent combination of education and experience.
