Job Description

Social Services About the Role We're seeking a compassionate and organized Social Services Assistant to support our residents and families in a skilled nursing environment. You'll work closely with the Social Services Director and interdisciplinary team to support residents' social, emotional, and psychosocial well-being while helping them navigate their care and transition needs. What You'll Do Assist the Social Services Director with daily department operations and resident services. Support residents and families with adjustment to the facility, illness, disability, and other emotional or social concerns. Maintain accurate and timely social services documentation and progress notes. Assist with care planning, discharge planning, and coordination of community resources. Provide information regarding Medicare, Medicaid, financial assistance, and available community services . Coordinate appointments and outside services, including dental, vision, podiatry, and psychiatric services. Communicate and collaborate with nursing, therapy, medical staff, administration, and other departments. Assist residents in achieving the highest practicable level of independence and well-being. Connect residents and families with appropriate community, health, and social service resources. Participate in resident and family meetings, staff education, and department training. Maintain confidentiality of resident information and comply with HIPAA, facility policies, and applicable state and federal regulations . Report suspected abuse, neglect, fraud, or unauthorized disclosure of protected health information as required. Qualifications Bachelor's degree in Social Work (BSW) required, or bachelor's degree in a related human services field with qualifying supervised social work experience, as permitted by applicable regulations. Minimum of 1 year of supervised social work experience in a healthcare setting preferred/required based on applicable regulations. Long-term care or skilled nursing experience preferred. Strong communication, organization, and problem-solving skills. Ability to work effectively with residents, families, staff, and community partners. Must meet all applicable state and federal requirements for the position. Social work licensure/certification as required by state regulations. Physical Requirements Ability to sit, stand, walk, communicate, read, and hear regularly. Ability to use standard office equipment and complete computer-based documentation. Ability to move throughout the facility to meet with residents, families, and staff. May occasionally lift or move items up to 25 pounds.
