This position requires comprehensive assessments, documentation in the electronic health record, and collaboration with team members and community agencies. Office hours are Monday through Friday, no weekends or holidays! Excellent salary and benefits including vacation time, paid holidays and employer matched 401K. This position is eligible for Federal Student Loan Repayment. Community Health Services is dedicated to providing a healthy environment for our patients, families and staff.
