Job Description

At Safe Opportunity Foster Alliance (SOFA) our mission is to provide safe, compassionate, and stabilizing short-term care for foster youth. We keep youth connected to their home communities while prioritizing their voice and choice in every decision. Through timely coordination, individualized services, and trauma informed care, we aim to reduce disruption, promote healing and prepare each youth for their best next step. The Child Liaison & Care Coordinator ensures that youth in SOFA's care receive comprehensive, coordinated services across medical, mental health, educational, and social systems. Acting as both an advocate and the hub of communication, this position helps youth and families navigate complex systems, ensures service plans align, and promotes trauma-informed support through collaboration with all stakeholders. Key Responsibilities Ensure each youth understands their rights, responsibilities, grievance procedures, and available supports. Assist youth in voicing concerns to staff and administration in a respectful and constructive manner. Support youth in building self-advocacy, communication, and conflict resolution skills. Act as a bridge between youth, their families/guardians, custodial agency and the treatment team. Provide families with timely updates regarding visitation, service planning, and youth progress. Serve as the central communication link among SOFA, custodial agencies, medical providers, therapists, and educational teams. Coordinate scheduling, referrals, and follow-ups for medical, educational, and therapeutic services. Track youth progress across multiple service plans and ensure alignment with SOFA programming. Anticipate and help resolve barriers to care (transportation, insurance, scheduling, access). Provide coordination and support during transitions of care and discharge support. Ensure services meet DCY and CARF standards. Provide quarterly reports for leadership summarizing youth concerns, systemic issues, and resolutions. Maintain accurate records in case files to support continuity of care. Uphold confidentiality in accordance with HIPAA, FERPA, Ohio law, and SOFA policy. Act as a supportive presence during disputes or crises, applying trauma-informed de-escalation strategies (Ukeru, CPI). Qualifications Minimum of Associates degree (Social Work, Nursing, Psychology, Education, or a related human services field preferred) 2+ years of experience in youth advocacy, case management, care coordination, or social services. Knowledge of child/adolescent development, trauma-informed care, and cross-system collaboration. Excellent communication and organizational skills, with ability to simplify complex information. Strong problem-solving skills and the ability to anticipate and address barriers to care. Must meet state requirements for background checks and hold a valid Ohio driver's license.
