Duties Conduct thorough patient assessments, including psychosocial evaluations and diagnostic evaluations, to identify individual needs and develop tailored care plans. Provide clinical counseling, including mental health counseling, behavioral therapy, motivational interviewing, and group therapy sessions for patients of all ages. Coordinate discharge planning and connect patients with community resources such as hospice care, assisted living facilities, or specialized services for developmental disabilities or dementia care. Collaborate with interdisciplinary teams to ensure holistic patient care through clinical interprofessional collaboration and adherence to social work practice standards. Maintain accurate medical documentation in electronic medical records (EMR) systems, including detailed case management notes and medical records compliant with HIPAA regulations. Assist with crisis intervention efforts for patients experiencing acute mental health crises or behavioral health emergencies. Support patients with complex needs such as addiction counseling, PTSD care, Alzheimer's care, or working with individuals from diverse cultural backgrounds. Skills Extensive experience in child & family counseling, hospice & palliative medicine, and working with geriatric patients. Proficiency in medical terminology, CPT coding, ICD-9/10 coding systems, and Medicare regulations. Strong knowledge of social work practice standards, clinical ethical guidelines, HIPAA compliance, and crisis management protocols. Skilled in case management, intake procedures, outpatient and inpatient social work practices across acute care settings such as hospitals and nursing homes. Ability to utilize EMR systems effectively for documentation of medical records and treatment plans. Excellent client communication skills with a focus on trauma-informed care and motivational interviewing techniques. Experience working with individuals with disabilities—including developmental disabilities and autism—and familiarity with special education practices.
