Job Description

New!! $ 5,000 Sign ON Bonus!!!! As Director of Social Services, you are delegated the administrative authority, responsibility, and accountability necessary for carry out your assigned duties. You are responsible for providing services to respond to the emotional needs of the residents and their families. The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of the Social Services department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and ensure that the medically related emotional and social needs of the resident are met and maintained on an individual basis. Job Duties and Responsibilities 1. Keep up-to-date on current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Executive Director. 2. Implement policies and procedures for the identification of medically related social and emotional needs of the resident. 3. Participate in discharge planning; development and implementation of social care plans; and resident assessments. 4. Maintain a quality working relationship with the medical profession and other health related facilities and organizations. 5. Conduct and document a social services evaluation, including identification of resident problems and needs. 6. Provide/arrange for social work services as indicated by resident/family needs. 7. Keep the resident's family informed of resident problems, personal needs, transfers, and changes of level assignment. 8. Meet with the appropriate facility staff concerning resident issues. 9. Ensure all documentation complies with applicable regulations 10. Act in compliance with all corporate, state, federal, and other regulatory standards. 11. Conduct oneself with the highest degree of honesty and integrity in every interaction. 12. Demonstrate a passion for caring as evidenced by interactions with co-workers, residents, families, and visitors. 13. Perform other duties, as assigned. Must possess, as a minimum, a Bachelors Degree, LSW or MSW from an approved school of Social Work. Must possess current license/certification in the state of practice. Minimum of one (1) year experience in long-term care preferred. $2,500 Sign-On Bonus
