Job Description:
The Social Service Worker provides psychiatric social work services for inpatient hospital clients for FMC and Psychiatric Health Services as part of a multidisciplinary team. Functions across a continuum of care by providing individual, family, and group therapy as directed and supervised by Psychiatrist. Collaborates with physicians and other members of the healthcare team to develop patient specific treatment plans. Monitors patients clinical progress throughout hospitalization through discharge. Provides ongoing assessment and consultation to meet patient and family needs. Assesses and provides educational instruction and materials to meet learning needs of patient and family members as related to each case and to healthcare providers relevant to particular cases. Completes psychosocial assessments. Completes multidisciplinary treatment plan. Assists with multidisciplinary assessment summary page for all discharges. Provides groups and individualized therapy as directed. Facilitates patient transfers to other psychiatric facilities. Coordinates with the psychiatric physician facilitation for judicial hearing needs. Coordinates post discharge needs.
QualificationsJob Qualifications:
Bachelors in Social Work with approval of Ohio Department of Mental Health permitted. Masters degree in Social Work preferred. Licensed Social Work (LSW) with practice directed and supervised by attending Psychiatrist(s) permitted. Licensed Independent Social Worker (LISW) preferred. A minimum of three years of experience or combination of experience and education. Exerting up to 20-50 pounds of force occasionally, and/or up to 10-25 pounds of force frequently, and/or a negligible up to 10 pounds amount of force often.
