Job General Summary/Responsibilities:

Support Facility:
Atrium Medical Center The medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient's social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and social work services are provided to inpatient and outpatient populations of the hospital.
Qualification:
Education Minimum Level of Education Required:
Master's degree Area of study or major : Social Work Licensure/Certification/Registration LSW required; LISW preferred Licensure with the
State of Ohio Counselor, Social Work and Marriage and Family Therapist Board Experience Minimum Level of Experience Required:
1 - 3 years of job-related experience Preferred experience: Previous hospital or community social work experience required. Knowledge of community agencies preferred Knowledge/Skills Excellent interpersonal skills, critical thinking, and ability to communicate in writing. Excellent organizational skills. Ability to work in a busy, stressful environment.
