This vital role provides flexible, as-needed psychosocial support to patients, families, and caregivers during one of life's most profound transitions. Key Responsibilities Conduct psychosocial assessments within 5 days of admission, identifying emotional, social, financial, and environmental needs. Participate in IDG meetings and collaborate with the interdisciplinary team to deliver whole-person care. Observe, record, and report changes in the patient's emotional and social well-being throughout the course of care. Assess the patient's and family's ability to cope with the dying process, offering meaningful support every step of the way. Evaluate the effectiveness of social work interventions and adjust approaches to meet evolving needs. Participate in discharge planning as needed.
