We are looking for a Social Services Designee (SSD) to join our skilled nursing and long-term care team! As our Social Services Designee, you will serve as an advocate for our residents and families while working closely with nursing, therapy, admissions, and the interdisciplinary team to ensure each resident receives the support, resources, and services they need. What You'll Do Assist residents and families with the social, emotional, and psychosocial aspects of care. Participate in resident assessments, care planning, and interdisciplinary care conferences. Assist with discharge planning and coordinate services necessary for safe transitions back to the community or another level of care. Help residents and families understand available community resources, benefits, services, and support programs. Advocate for residents' rights, preferences, choices, and quality of life. Address resident and family concerns and assist with resolving grievances in a timely and professional manner. Support residents adjusting to admission, changes in condition, room changes, loss, and other significant life transitions. Assist with advance directives and related resident/family education as appropriate. Coordinate with hospitals, physicians, hospice, home health, community agencies, transportation providers, and other outside resources. Complete required social services documentation accurately and timely. Participate in facility meetings, care conferences, QAPI activities, and other interdisciplinary initiatives as assigned. Maintain resident confidentiality and comply with HIPAA requirements. Follow all applicable federal and Ohio requirements, facility policies, and resident rights regulations. Build positive relationships with residents, families, coworkers, and community partners. What We're Looking For Experience in skilled nursing, long-term care, healthcare, social services, case management, or a related setting preferred. Knowledge of the psychosocial needs of elderly and disabled individuals preferred. Strong communication, organization, problem-solving, and documentation skills. Ability to work effectively with residents, families, nursing staff, physicians, therapy, and other members of the interdisciplinary team. Compassionate, professional, and resident-centered approach. Ability to manage multiple priorities in a fast-paced healthcare environment. Must meet all applicable CMS and State of Ohio qualification requirements for the position. Why Join Our Team?
