A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description

Liberty Health Care Center is accepting applications for a full time Social Services Coordinator. Due to a few movements in our company, we have a few opportunities for people that are passionate about social services and being a resource for our residents!
Job Duties:
Acts as a liaison between the resident, family and staff of the facility for any concerns, which may arise. Initiates an admission interview and assessment Determines the Social Service needs and/or interventions and develops a Plan of Care. Monitors for needs and arranges for Dental, Hearing, vision, podiatry, psychological screenings and assists physicians in building. Participates as part of the Interdisciplinary team in maintaining a Plan of Care. May be required to coordinate Interdisciplinary team and family meetings. Participates as part of the Interdisciplinary team in providing interventions to resolve behavior or mood problems. Attends Resident Care Conferences and completes appropriate section in MDS with signature. Assures that the resident is aware of Resident's Rights. Assists them in exercising their rights if they desire. Facilitates and coordinates Discharge Planning to assure continuity of care. Arranges for necessary home health care and necessary equipment. Maintains a current list of Community resources. Assists the residents in their transportation needs for appointments. Encourages residents in maintaining or attaining the highest practicable level of functioning possible. Coordinates Hospice Services as needed. May be required to attend resident council meetings
Qualifications:
At least one year experience in Social Service work in a long term care setting preferred. Must be willing to work flexible hours. Must have excellent communication skills, written and verbal. LSW or a degree in social work preferred, a degree in similar field with experience on long term care will be considered.
