Job Description

We are seeking an experienced Clinical Social Worker / Medical Social Worker to join a hospice care team serving patients and families throughout the community. This position provides compassionate psychosocial support to patients and their families during the final stages of illness. Services are provided in a variety of settings, including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes . Clinical Social Worker -
