Full Time Key Responsibilities:

Assist with discharge planning, patient placement, and referrals for Swing Bed patients. Provide emotional and therapeutic support to patients and families. Coordinate referrals to mental health, substance abuse treatment, tele-psychiatry, and other community resources. Maintain accurate and timely case management and service documentation. Assess resident needs and participate in care planning and resident care conferences. Assist residents and families with Advance Directives. Advocate for resident rights and individual needs. Participate in multidisciplinary meetings, quality improvement initiatives, and patient-centered activities. Develop and maintain professional relationships with community mental health agencies and other service providers. Collaborate with Hospice and assist with home visits when needed. Support clinic staff with social work needs as appropriate. Maintain professional relationships with patients, residents, families, staff, community partners, and the public. Perform other related duties as assigned by the Chief Nursing Officer.

Required Qualifications:

High School Diploma or GED equivalent. Bachelor's degree in Social Work (BSW). Basic Life Support (BLS) certification through the American Heart Association (AHA) — must be obtained within 60 days of hire. Valid driver's license or permit.

Preferred Qualifications:

Experience in a critical access hospital, long-term care, or hospice setting. Master's degree in Social Work (MSW). Licensed Clinical Social Worker (LCSW).

Pay:

$80,000.00
