A Family, School, or General Case Manager coordinates the delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs. Provide limited counseling to client and their family.

PURPOSE:
The Behavior and Disabilities Coordinator supports the coordination of behavioral, mental health, and disabilities services for children enrolled in the Early Head Start and Head Start programs. This position coordinates documentation, referrals, data tracking, and communication among center staff, families, service partners, and the Behavior and Disabilities Supervisor in accordance with Head Start Program Performance Standards and Agency policies.
ESSENTIAL JOB DUTIES
I.
DISABILITIES SERVICE COORDINATION A.
Provide administrative and coordination support for disabilities and special education services. B. Organize and maintain disabilities service materials, forms, screening tools, and resources. C. Request, track, and process release of information (ROI) parent/guardian authorization forms. D. Track and coordinate Individualized Family Service Plan (IFSP) team meetings and related communication among EI/ECSE partners, the Behavior and Disabilities Supervisor, and center staff. E. Monitor completion and entry of Teacher IFSP meeting summaries in the child's file and communicate needed follow-up to designated staff. F. Monitor reports for expiring IFSPs and ROIs. G. Prepare EI/ECSE referral documentation for review and approval by the Behavior and Disabilities Supervisor. Submit approved referrals to the EI/ECSE agency, track referral status, and keep designated center staff informed.
II. BEHAVIORAL AND MENTAL HEALTH SERVICE COORDINATION A.
Send, receive, enter, and monitor DECA forms for children with guidance plans. B. Draft and update guidance plan documentation, as needed, under the direction of the Behavior and Disabilities Supervisor. C. Enter and maintain guidance plan meeting documentation, as needed, to support Education Coaches and the Behavior and Disabilities Supervisor. D. Coordinate the organization, maintenance, authorized distribution, and placement of Mental Health Consultant documentation in designated child files; identify missing or incomplete documentation for follow-up with the Behavior and Disabilities Supervisor. E. In partnership with the Behavior and Disabilities Supervisor, coordinate Mental Health Consultants' access to approved camera systems when a virtual observation is requested by a site.
III. DATA, DOCUMENTATION, REPORTING, AND DEPARTMENT SUPPORT A.
Maintain accurate and current behavioral, mental health, and disabilities records in designated child files, data systems, and departmental tracking tools. B. Monitor records and reports for missing, incomplete, or expiring documentation and communicate required follow-up to designated staff. C. Gather and enter service data and generate routine department reports for review by the Behavior and Disabilities Supervisor. D. Assist with preparing documentation and reports required for internal monitoring, program reporting, and compliance review. E. Prepare department-specific training materials, forms, brochures, and community resource information for distribution to centers, staff, and families.

REQUIRED JOB SPECIFICATIONS
Education:
Bachelor's degree in early childhood education, special education, child development, psychology, behavioral health or a closely related field required .
Experience:
Minimum two years of experience in early childhood education, special education, disabilities services, behavioral or mental health services, or related field required . Experience coordinating services for children, working with EI/ECSE and IFSP processes, maintaining confidential child records, or supporting Head Start/Early Head Start programs preferred.
Essential Skills:
Keyboarding and computer proficiency to include Microsoft Office Suite and database programs. Excellent interpersonal, written and verbal, communication skills; proficient mathematical abilities; proficient filing and sorting skills (alphabetical and numerical). Ability to perform job duties with a high level of accuracy. Ability to read, write, and communicate effectively in English to understand and follow procedures, ensure safety, and complete required documentation related to the position. •Bilingual staff must possess sufficient English/Spanish language proficiency to facilitate translation of verbal and written data in all forms and ability to provide accurate translated information.

Essential Physical Requirements:
This position is classified as Administrative/Office‑Based. Ability to occasionally lift and carry up to 15 lbs. a distance of 15 feet. Ability to sit and/or stand for extended periods of time, up to 8 hours per day. Sufficient mobility, strength, and range of motion to occasionally stand, stoop, kneel or squat, reach, twist, and bend.
