Job Description

The Addiction Counselor will provide counseling and support to individuals struggling with addiction, helping them navigate the path to recovery. Prison Discharge Planner / Corrections Social Worker / Corrections Counselor / Drug Recovery and Resettlement Looking for a Social Worker, Human Services, Criminal Justice or Mental Health Professional with addiction experience to work directly with prisoners that have a history of drug abuse, assist with discharge planning, MAT therapy and appointments, coordinating community resources are in place for reentrants on the date of their release.

Job Duties:
Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment. Assist with referrals in the community, including setting up and scheduling appointments for medication and treatment. Ensure needs, benefits, and services are obtained in accordance with assigned individuals' treatment plans. Track and update spreadsheet/database on everyone released from the facility Meet with reentrants with positive urine drug screens to assess and refer to treatment. Complete assessments as required (Initial, 6 months, discharge). Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.
Requirements:
Bachelor's in Social Worker or Human Service or Similar field required. Knowledge and experience in Case Management, Discharge Planning and Community Resources. Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families. Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.
